Functional electrical stimulation cycling promotes recovery in chronic spinal cord injury
Mar. 4, 2013 ? A new study by Kennedy Krieger Institute's International Center for Spinal Cord Injury (Epub ahead of print) finds that long-term lower extremity functional electrical stimulation (FES) cycling, as part of a rehabilitation regimen, is associated with substantial improvements in individuals with chronic spinal cord injury (SCI). Improvements include neurological and functional gains, as well as enhanced physical health demonstrated by decreased fat, increased muscle mass and improved lipid profile. Prior to this study's publication today in the Journal of Spinal Cord Medicine, the benefits of activity-based restorative therapy (ABRT) programs, such as FES cycling, were largely anecdotal despite publicity in conjunction with the recovery of actor and activist Christopher Reeve.
In FES, small electrical pulses are applied to paralyzed muscles to stimulate movement. In the case of FES cycling, FES pulses prompt the legs of an individual with SCI to "cycle" on an adapted stationary recumbent bicycle. The repetitive activity offers cardiovascular exercise similar to that which an able-bodied individual achieves through walking, but this new research shows that the results go far beyond basic health benefits.
"Exercise has not been commonly advocated for individuals with paralysis because of the assumption that it is of little benefit and it is challenging to exercise limbs that an individual cannot voluntarily move," said John W. McDonald, M.D., Ph.D., senior study author and director of the International Center for Spinal Cord Injury at the Kennedy Krieger Institute. "However, we found that FES cycling is a practical form of exercise that provides substantial benefits, including improved physical integrity, enhanced neurological and functional performance, increased muscle size and strength, reduced muscle spasticity and improved quality of life."
Participants in the study included 45 individuals diagnosed with chronic SCI, defined as paralysis continuing more than 16 months following injury. Twenty-five patients with chronic SCI were assigned to the FES cycling group at the Washington University Spinal Cord Injury Program, where Dr. McDonald was section head until 2004. These patients were matched by age, gender, injury level/severity and duration of injury to 20 control patients who received no active physical therapy.
On average, patients did FES cycling for 29.5 months, with an average distance of 10,466 complete cycles of the two pedals per week, the equivalent of taking 20,931 steps. In the retrospective analysis conducted by Kennedy Krieger researchers, improved motor function was observed in 80 percent of the FES group, compared to only 45 percent of control subjects. Clinically important gains in neurological function were also observed in the FES group; response in pinprick sensation was observed in 56 percent of the FES group compared with 25 percent of the control group, while 14 of the 25 FES subjects showed response in light touch scores compared to six of the 20 controls.
Results also showed that FES cycling enhanced muscle strength without increasing spasticity, a common side effect of paralysis that varies from mild muscle stiffness to severe, uncontrollable leg movements. The FES group was found to be on fewer anti-spasticity medications with lower doses than the control group, suggesting that the lower level of spasticity observed in the FES group was not due medication differences between the participants.
FES cycling was also associated with improvements in quality of life and correlated with overall improved health. FES and control groups showed no significant difference in total thigh volume; however, total thigh fat, measured by MRI, was 44.2 percent less in the FES group than in the controls.
According to Dr. McDonald, this is a key finding because intramuscular fat is associated with glucose intolerance, a complication affecting nearly two-thirds of individuals with chronic SCI.
The results of this study support the hypothesis that activity-based rehabilitative strategies can play an important role in promoting physical integrity and functional recovery, even when implemented years after an injury, and provide rationale for a large prospective randomized clinical trial to evaluate the efficacy of ABRT using FES in persons with chronic SCI.

Cash airlift helped avert Greek bank run during debt crisis: paper
ATHENS (Reuters) - Greece's central bank had billions of euros of banknotes shipped in from other central banks to avert a bank run during the country's debt crisis as depositors withdrew their money, newspaper To Vima reported on Sunday.
Fears the debt-laden country might ditch the euro and return to the drachma led Greeks to pull out billions of euros of savings in the last three years, stashing their cash under mattresses or in safe deposit boxes.
"While many talked about a lack of liquidity in the economy, the cash circulating ... had no historical precedent," the paper said, citing central bank officials it did not name.
Central bank officials could not immediately be reached for comment.
Cash in circulation jumped to 48 billion euros ($62.32 billion), about a quarter of gross domestic product (GDP), in June 2012, from 20 billion before the crisis erupted, way above normal ranges of 6 to 8 percent of GDP in developed economies, the paper said.
To meet the high demand for cash, the Bank of Greece had loads of banknotes secretly airshipped from abroad, feeding it to banks throughout the country to avoid shortages that could have intensified worries and set off a bank run.
The Bank of Greece only prints 10-euro banknotes.
Greek banks lost about a third of their deposits after the debt crisis exploded in late 2009. Without access to wholesale funding markets, they came to depend on the central bank for their liquidity needs.
However, uncertainty has decreased over the country's future in the euro zone after a second round of elections in June last year, which led to a three-party pro-bailout coalition government and a return of deposits to banks.
Official data released last month showed 10.4 billion euros had come back to the system. Business and household deposits stood at 160.9 billion euros at the end of January. ($1 = 0.7702 euros)

Angels' Trout takes high road on salary, position
TEMPE, Ariz. (AP) ? Los Angeles Angels slugger Mike Trout figures if he keeps producing like he did during his historic rookie season, he'll be in line for a handsome raise soon enough.
The Angels renewed the contract of the AL Rookie of the Year for $510,000 on Saturday, just $20,000 above the major league minimum, prompting an angry response from Trout's agent.
While Craig Landis said the renewal "falls well short of a 'fair' contract," Trout took the high road before a morning workout at the Angels' spring training complex on Sunday, repeatedly insisting that "I'm just happy to be in the lineup."
"I mean, my time will come," Trout said before a team meeting. "I just have to keep putting out numbers and concentrating on one thing, and that's getting to the postseason."
Trout had a base salary of $482,500 last year, when he hit .326 with 30 homers and 83 RBIs, and led the majors with 129 runs and 49 steals. Trout was the run-away choice as the AL's top rookie, earning a $10,000 bonus, and finished second to Detroit's Miguel Cabrera in MVP voting.
Trout has 1 year, 70 days of major league service, which the Angels historically have place more weight upon than performance in renewing contracts. Teams are allowed to renew the contracts of unsigned players on their 40-man rosters from March 2-11.
There were 22 players whose contracts were finalized by the Angels on Saturday. The highest salary of those players went to first baseman and outfielder Mark Trumbo, who will make $540,000.
Trout likely will be eligible for arbitration after the 2014 season.
"During the process, on behalf of Mike, I asked only that the Angels compensate Mike fairly for his historic 2012 season, given his service time," Landis said in a statement. "This contract falls well short of a 'fair' contract and I have voiced this to the Angels throughout the process. Nonetheless, the renewal of Mike's contract will put an end (to) this discussion."
Nationals outfielder Bryce Harper, the reigning NL Rookie of the Year, will make $750,000 this season under terms of the five-year deal he signed through 2015.
Harper hit .270 with 22 homers, 59 RBIs and 18 stolen bases in 139 games last season.
Landis also made it seem that Trout was unhappy with a move to left field this season, which was designed to allow speedy Peter Bourjos to become the Angels' primary center fielder.
"As when he learned he would not be the team's primary center fielder for the upcoming season," Landis' statement said, "Mike will put the disappointment (of his salary) behind him and focus on helping the Angels reach their goal of winning the 2013 World Series."
Trout acknowledged Sunday that he prefers center field, the position he's played since the Angels selected him in the first round of the 2009 amateur draft. But he also said that he believes playing in left field will help him to become a more complete outfielder.
"I'm a center fielder, obviously," Trout said. "But you know, when you're an outfielder, you should be able to play all three. I think it's going to help me get reads off the bat. It's going to be a fun adjustment for me."
During batting practice this spring training, Trout has made a habit of spending two days in left field and two days in center, often rotating on a daily basis. He said the idea was to remain sharp in center field while adjusting to left, where he mostly played late in games last season.
"My main position is center field, obviously. It's definitely a different position than left field. But I just have to make an adjustment and go with the move," he said. "I feel fine out there. Just getting into games, getting some experience out there will definitely help me."
Angels manager Mike Scioscia said Trout still will play center field, and that his versatility ? his ability to play left ? is one of the reasons he is so valuable.
"Mike is going to be a center fielder, no doubt. We understand that's what's his position," Scioscia said. "But right now his versatility is something that's going to make us a better team, and he will play some center field this year. He's going to play left field, too."

SEFFNER, Fla. (AP) ? Officials have called off the effort to recover the body of a man swallowed by a sinkhole below his Florida home.
Hillsborough County Administrator Mike Merrill said Saturday that they were ending the effort to Jeff Bush's body.
Bush was in his bedroom Thursday night in his home about 15 miles east of Tampa when the earth opened and took him and everything else in his room. Five others in the house escape unharmed.
Merrill says officials planned to bring in heavy equipment on Sunday to begin demolishing the home.
He says "we're dealing with a very unusual sinkhole."
